Losing my ealth insurance due to disability in Il cannot work what to do

29 years old with chronic back pain with disk herniation. Out of work for 8 months and doctor is not releasing me to return to my job. Exhausted my short term disability insurance and cannot get on long term as my back was pre existing condition. Where can I get insurance?
