As a part of most health insurance applications there is a series of questions you must answer pertaining to your medical history. Even applications for health plans that are not medically underwritten may ask health questions in order to analyze pre-eisting medical conditions for eclusion periods or rate adjustments. An employer application for Small Business may also ask health questions pertaining to the overall health of the employees to be enrolled under the group health plan. For small Small Business, this information may only be used to determine premiums based on a rate adjustment factor. Individuals applications are usually more detailed about the information they require on the application. This is because, in most states, the insurance company will use this information to determine if you are a reasonable risk. If, based on your answers, the insurance company decides that insuring you will result in ecessive claims payments, your application may be denied. When applying for more than just one person in the family, the health questions will apply to all persons wishing to be covered under the health plan. The amount of health information you must provide on the application will depend on the type of coverage you are applying for and the underwriting policies of the insurance company with whom you are applying.
